Wind, falling branches, foot traffic from satellite installers or HVAC techs - any of it can crack a tile or break the foam adhesive bond and slip a tile out of position. One slipped tile becomes a leak the next storm.
The mortar bedding under ridge caps and hip caps shrinks and cracks over a 15 to 25 year span. Wind drives rain past the cracked mortar and into the underlayment. The roof looks fine from the ground while water is wrecking the deck underneath.
Valleys are the most stressed area of any tile roof. The lead or aluminum valley flashing under the tile fatigues and cracks. Water that should run down the valley starts running under it. This is the silent killer of older tile roofs.
Plumbing vent boots, attic vents, and skylight flashings on tile are different from shingle. They need lead or specialized engineered flashings. Cheap rubber boots crack under Florida UV in 5 to 8 years and start leaking.
Honest answer first: if your tile roof is under 25 years old and the deck and underlayment are sound, repair is almost always the right call. The tiles themselves last 50-plus years.
The thing that fails first is the underlayment - the felt or synthetic membrane under the tiles. When the underlayment fails across the whole roof, you do a tile lift-and-relay: pull the tiles off, replace the underlayment, put the same tiles back. The tiles themselves are still good. You save a huge portion of the replacement cost.
When the tiles themselves are spalling, delaminating, or repeatedly cracking, then full replacement makes sense.
